Our beloved mother, grandmother, and great grandmother Rama Patel, 92, passed away peacefully and surrounded by family on Saturday March 20th. She lived with three generations of her united family in Kanata - a greater testament than any of the love and respect with which she was showered and the values she instilled in her family. She was full of superhuman will, energy, ethic, love, devotion, wisdom, discipline, and spirit. She was our Iron Lady - with courage of her convictions to blaze her own path from a young age. She was a trend-setting seamstress, street-smart self-made entrepreneur, and a respected volunteer Gujarati teacher for many years. She was a loyal friend and ‘ba’ to many in the community. She lived her life fully and without regrets. She traveled the world, enjoyed cooking and sharing meals, and was always elegantly dressed. Age was just a number: as recently as last year, she’d regularly go swimming or to the Bayshore mall on her own. Ever a prudent punctual perfectionist, she kept those around her on their toes with her tough love and cultivated a habit of excellence. She devoted her life to raising her family. She was a dutiful daughter, a selfless mother, and a doting grandmother, sister, aunt, and friend. She lent a hand to those in need and stood by them in their sorrows. Her trailblazing story is an inspiration to us all and a lesson that even during extreme adversity, one can live with great pride, honour, independence, belief, generosity, and dignity. She was the best mother one could have. She embraced all the life’s ups and downs, joys and sorrows as divine will. Through thick and thin, she maintained her sense of humour - she laughed easily, often, and loudly. Her deeply held faith was a bottomless source of strength. She forged ahead with grit and equanimity. She never, never, never shied away from her duty. She never delayed, dithered, or gave excuses. She embodied and lived a life full of dharma and karma. Her actions spoke louder than words. She wasn’t just great, she was good. She’s survived by her children (Amrish and Smita in Kanata and Manoj and Sonal in Vadodara), four grand kids, and four great grandkids. She will be missed greatly and will live forever in our hearts. She remains our superhero - a triumph of self-belief and hard work over adversity. She always chose the right path, not the easy. Till her last smile, her will to fight with her head held high influenced all those around her. She gave us the best of her. We are proud and lucky to call her our mother. We pray to Bhagwan to grant us strength as we endeavour to follow her on the path of grace and “be like ba”. Please join us in praying for her moksha. Om shanti. The family would like to thank - from the bottom of our hearts - the staff at the Queensway Carleton hospital, friends, family, and community for all their generous support over these past few weeks. In lieu of flowers, we suggest a donation to the Queensway Carleton Hospital, ACE Unit in her memory. My Tribute Gift Foundation makes this online charitable giving portal web page MyTributeGift.org (the “Website”) available to you subject to your acceptance of and compliance with these terms of use (the “Terms of Use”). Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
